All clients are required to fill out this form before attending the first music therapy session.

The client's teacher, doctor, therapist or any other professional that works with the client can use this form to refer the client to music therapy.

The music therapist uses this form to determine the client's strengths and potential goal areas. Assessments are conducted once every three months.
